Good Faith Estimate
The good faith estimate (or GFE) is a notification that outlines an uninsured (or self-pay) individual’s expected charges for a scheduled or requested item or service. Providers and facilities must give this estimate to an uninsured (or self-pay) individual (or their authorized representative) who requests it or who schedules an item or service. The good faith estimate will also include items or services reasonably expected to be provided along with the primary item(s) or service(s), even if the individual will receive the items and services from another provider or another facility.
Lone Star Chiropractic’s policy is to provide all new patients estimated charges for all recommended services including any estimated insurance benefits during your care
- Make sure your health care provider gives you a Good Faith Estimate in writing at least 1 business day before your medical services begin. You can also ask your healthcare provider, and any other provider you choose, for a Good Faith Estimate before you schedule an item or service.
- New patient services for consultation, examination, and x-ray will not exceed $140.00.
- Our adjustments will not exceed $50.00.
- Routine office visits (excluding exams, laser) will not exceed $80
- Make sure to save a copy or picture of your Good Faith Estimate given to you on your second visit (report of findings). If you receive a bill that is at least $400 more than your Good Faith Estimate, you can dispute the bill.
